"waters off" is a wrong usage.It should be "waters of". Let us leave this since it is the same in all the answer choices.
We can easily eliminate the answers with conjunction "and".
The options left are A,D and E.
Option D is awkward - "had generally found"
Between A and E, I will choose A because "generally found" is better than "found generally".

The answer is E. Its a direct case of violation of the parallelism rule. How items.... should be logically connected with how frequently... In D, we have the 'how..' but the ending is messed up. It goes into a different tense. E puts it correctly. Am i right??

Tha answer is E as a,b,c is out as these are parellel with main clause how items are placed on shelves and in d though there appears how in subordinate clause but verb usage is wrong turned should be turns follows the parellelism rule hence e is the answer

What is 'turns' parallel with? Shouldn't 'turned' be parallel with 'placed'?

Simple way to look at parallel structure when using AND is evaluate both the items Separately.

First part of the sentence.
It is well known in the supermarket industry that how items are placed on shelves is crucial to profits
Second Part of the sentence:
Using option D:
It is well known in the supermarket industry that how frequently is the inventory turned over is often crucial to profits
Doesnt sound good...
Using Option E:
It is well known in the supermarket industry that how frequently the inventory turns over can be crucial to profits
Looks Perfect.

Another difference is D introduces OFTEN. Intended meaning of the sentence is CAN BE. Hence there is a shift in meaning also in D.

I go with "B". First I contemplated between B and C.They both maintained parallelism.However, C bungles the logic.
