here are my inputs
'That' is ' necessary so A and B are out.
C states that ' spending' time is parallel to 'require doctors' which is incorrect .
in D , ' spending' is continuation to 'require doctors to see more patients' .
So D states that plan imposes doctors to see more patients and spend less time with each patient.

I would explain this as follows

imposing and requiring or else impose and require (Well thats not the issue :D)

now if i include 'spending' along with an 'and' it becomes a part of the list....though it isn't :dontgeti:...thus an 'and' before 'spending' is not required

There's only one answer choice without and before spend/spending...thats D.....Simple as that...ther's actually no need to do a BHEJA FRY with parallelism and the rest:hurray:
