I have a PDF form of MFC Form. I have few question regarding this:

1 - There is an Acknowledgemnt part.. that would be filled by me or by the person who recieved appllication form? [I believe it would be filled by them. It says "Acknowledgement
Received an Application Form from _MY NAME_______
for admission to MFC Programme (Session 2009-2011)
Signature ______________________
Date __________________________"

2 - Regarding Extra-Curricular activities: I was the President of my college. Can i mentioned it here?

Don't bother about the acknowledgement part, that is useful only if you submit the form over the counter, the MFC office otherwise does not bother to send the ack. back to postal candidates. It will cost them 5 bucks postage.
Leave the part blank, that is for office use.

For any extra-curris, please note that you need to back these with documentary proof during PI, so if you write that in the form, please have the necessary docs in hand, or else you'll be in trouble during PI.

How did you manage to get the correct PDF? I tried on the day it was uploaded on the site, but it gave me two errors, so had to send in a request for a hard copy instead:

1. The header was: MFC Admission;'2008-10', should have been 2009-11

2. In the parent details, probably point no. 2, the form did not mention father's name, just the mailing address, inspite of my adding father's name.

Since I could not have edited the downloaded form as it would appear shabby, I was forced to send in my draft for a hard copy.

Lols.. You are right. but i tried to download the form very first day and the form had lot more other errors (specially with dates). Also, earlier it was not showing the parents address but only the Name. Also, Jaipur center was not in the list.

Now, it is showing the address twice in Parents Name & Address field and on the last page it says 2008 in the header.

They are rectifying the errors and may take few more days. IT Skill problems.

I don't see what the issue is about really, last year they upped the same form, so this year too it could have been the same.. there's really no need for them to experiment.
