one DD will be enough. just 1000 rupees, (so apply one get one free) put everything in a big envelope since u need to also put in two + two self addressed envelopes of standard size) write the same dd number on both the forms and write the date as of when u had taken the dd, not the date when ur filling up the form. dont forget to attest the affidavit. no marksheets required. if u want to be considered under some category u need to attest a copy of the proof also along with the form (like SC/ST etc). u also have to attest the photo on the form. u also need a photostat of your CAT admit card (write the register number on form). that is all. The declaration to be made by the candidate and his/her parent/guardian and should be signed and dated before the application is submitted

wat does dis mean?declaratn means dat last column of form no????no seperate document to be attachd?
4)m a general candidate--so all i hav 2enclose is cat admit card photocopy n DD,right??n 2self adresd envelops,accordin 2u......

affidavit has to be submitted at the time of admission
and moreover everything is specified in the instructions
come on guys u r gng to be future managers , these type of queries regarding only form filling dont suit u at all , am i right
please first read all the instructions carefully then ask us if still left with any query we are there to help u out

It was clearly mentioned in the USB Handbook Information2009 pdf file, about the submission of the documents:

SUBMISSION OF DOCUMENTS
1. Last date for submission of completed Application Forms to the Chairman, University Business School, Panjab University,
Chandigarh is November 15, 2008 by 5.00 P.M.
2. The candidates under reserved categories and under Foreign Nationals/NRI category will submit the necessary certificate(s)
required by the University in the prescribed proforma given in the Handbook of Information at pages 12-13.
3. In case of gap of one or more sessions between passing the last examination and admission being sought now, a candidate
shall have to give an affidavit concerning not having been involved in any offence/unlawful activities, attested by the Magistrate
1st class.
4. The students who have passed the qualifying examination from an institution other than Panjab University, will be eligible, only if
such examination/degree is recognised as equivalent to corresponding examination/degree of the Panjab University


Just see the 3rd point, we've to send the affidavit along with the application form and not as specified above at the time of admission.

2) 2 self addressed envelopes are compulsory.
3) No separate document must be attached. Just sign the form at the specified places.
4) Make sure that you don't forget the affidavit, if you have a gap of 1+ year.

u can attach the affidavit along with the form but for ur knowledge we all deposited affidavit and migration certificate at the time of admission
it is not mentioned over here that u have to submit it along with the form, its just written that for taking admission u have to give this document
dont forget to keep a photostat copy of that affidavit
also i will confirm the things which are to be attached along with the form from our admission department tomorrow and tell u all

We need to attach a photocopy of the admit card..but i haven't received mine yet...what can be done??do i need to wait till a get it??
I had passed out in 2007 and i have been working since then..so do i need to send this affidavit??
The photograph has to be self attested or by a notary??

1) you can wait for 2 more days as last date to receive CAT Admit card is Oct 31st, 2008...

2) and ya u need to give affidavit even if u r employed. read the below written line once again:-
"In case of gap of one or more sessions between passing the last examination and admission being sought now, a candidate

"

3) photograph shd be attested by notary not just self-attested..

1 doubt puys,

I bought the handbook application from Panjab University itself and it contains 2 forms (ludhiana & Chandigarh) plus one brown color big envelopee and 2 small ones...

Now, I dont want to apply for Ludhiana college. So, I just need to fill Chandigarh form and give my complete postal address on those small envelopees and send it across... thats it??? am i right??? i have already taken xerox of my admit card and affidavit is also ready.. :D

How abt Ludhiana Form??? can i simply throw it as i dont wanna apply to the college... I hope it wont hurt their EGO and they dont mind in case that form is missing...
