I don't think that the CAT forms are available online...you will have to take the application form from the AXIS bank.
For ur information, here are the list of important dates for the CAT application.
 Important Dates - Issue of CAT Bulletins by Axis Bank STARTS :
JULY 14, 2008 (Monday)
- Issue of CAT Bulletins by Axis Bank ENDS :
AUGUST 8, 2008 (Friday)
- Issue of CAT Bulletins by IIMs ENDS :
AUGUST 13, 2008 (Wednesday)
- Last date for receipt of completed CAT Applications by the IIMs : SEPTEMBER 5, 2008 (Friday)

Hi guys , i am also preparing for cat 08 , and as i am also a working professional , i dont get much time to study , i also study hardly one hour a day , which i think is not enough , but still something is better then nothing , .....

Hi friends,
i have one query.Thought to clarify with you puys.
I appeared for the first very mock test in the month of February and my score was 92 percentile.
But I am not in practice since 1st June due to some personal formalities and professional life.
And so now my score has decreased too much with clearing cut-offs in none of them.
But now I have started with the practice again.
Do you think with so much less time I may be able to make up to it with daily practice?
And also if my lack of practice for this 1.5 months the reason for my depreciation in the scores.
Kindly help.

hi PS,
this is a known problem for us, so called working professionals. In start of my preparation, i too faced the same problem. but i managed to take 2-2.5 hr in weekdays. then on weekends i can cover the topics by giving some extra time. usually i go to office on weekends with my prep books. since there is no seniors to watch me and no room-mates to make disturbance in study :idea:. i don't know exactly about your situation but think this can help.

Thereafter.. if you are in the morning shift and come home by around 7-8, take a break of around an hour and start working on your weak areas of QA and DI. (you can figure out your weak areas by giving Mock cats).
And before sleeping, read a good non-fictional book for about an hour.
Then you can devote the whole of weekend for just studying all the 3 sections and practice them.
I don't think any working professional would require more time than this as CAT does not test your cramming abilities. It only checks your logic provided you have your basics clear. I can say this because there are friends of mine who are in IIMs with having just prepared (read practiced QA and DI) just a month before CAT. But they had their basics in maths very clear (10th std) and had a good reading habit.

Don't ask this question to us. Ask this to yourself that whether you are ready to cover up what you could had in 1.5 month. And if you have the passion then the answer will be yes.
I believe never think that you have not much time left now. Whenever you start just give your full efforts.
The drops in score may be because you are not in touch but then you have reason to come back (as you have scored 92%)

Hey hav u guys bought the CAT form?? i hav noticed that therz a 'Fold along this line' in the OMR sheet of the CAT form, but the OMR has been folded not along this line but slightly above..and the fold line is passing thru the option bubbles....  anyone else noticed it??
Anyone can give up, it's the easiest thing in the world to do. But to hold it together when everyone else would understand if you fell apart, that's true strength

i believe that won't create a problem...as far as the bubbles are readable correctly... 
even mine was the same case...i sent it as it is...
