I bought the form from IMS center..it has the brochure and the form...do we need to send nething else along with this ?? Once this is filled n sent out, is there ne online process required for completion of application ?

Its either online application where u pay through card or u fill the hard copy and send them along with the DD. Call office @ 04312520502

By last week of jan or first week of feb u can check the website.
