I will tell you what I understand from the whole paragraph. :splat:
There is this doctor who has some unusual hands 😛 .. So, the para starts by saying, in general, patients look for some qualities in the hands of the surgeon.. But with this doctor, the scene is different.. even before the patients know it, the doctor gets to know that the patients are checking his hands out.. and they do it by "the downward glance repeated, the prepared questions beginning to falter, the overemphatice thanks during the retreat to the door" .. Second last sentence says - The doctor knows before the patients do - " ", but he is not that concerned..
