In the dashboard instructions the 3rd point says there's a common form to fill which is mandatory.Any idea where this common form is ? Only seeing intitute specific form on dashboard(one with family details,doc upload etc.)

While uploading the SNAP admit card, ensure that the name and photograph are clearly visible. If a candidate is not able to produce the same, he/she has to sign an affidavit form in the format provided by SIBM Pune.
