What has to be written in the field marked as 'Module' , in the application form of the common admission process?

Module refers to your interview slot i.e Forenoon/Afternoon
Class/Division- 1st Division if score of 60% or more.
Extra Curriculars- Non academic achievements eg. sports, media etc
Co-Curriculars- Academic achievements eg. merit holder, Olympiad etc

1. Format of the Dates to be mention in Graduation column.
2.If you are appearing for the final year Bachelor's degree, If not then NA??
3.*Please bring the total experience and the copies of all salary slips at the time of interview. : I have total experience certificate from my previous organization.

In data from they have asked for all salary slips ... I have experience of 40 months so do I have to take all 40 pay slips ??
