While submitting the form i get the msg "Query Admission Failed" Help. TIA

I have calls from IIM C, L, I and K. Also I have been short listed for all new IIMs. I know that I don't have to give separate PI for new IIMs. But is there any form I have to fill up and submit for the same?
The CAT 2015 website shows a Candidate Identification Affidivit in Certificates & Proformas. Do we need to submit that too??

I did my ug summer internship in Shanghai. Can I mention it as co-curricular activity ? I have certificate for the same. Also I took part in a marketing event in current college. I have a pdf having brief of the event and name of team and its members, including my name. Will that suffice for proof ? Please help.

Yesterday when i checked the shortlist, it said congrats you are shortlisted. But when i checked now for filling the form, it says we regret to inform you that no data is available with this registration no. Wtf is this??

guys in "Marks considered for award of Class/Division in Bachelor's degree" block they r asking for third year marks...but i'm in 3rd year right now 6th sem...without entering third year marks they r not allowing to submit the form......wht to do now ??
