1) yes, "that" refers to the preceding noun, but remember "of" is jst a middleman. you need to read the sentence taking off the middleman.And that makes "patchwork" the subject of the sentence.
2) i doubt if the OA is A. The subject here is "patchwork" and thus a singular verb is required. "surrounds". Looks OA to be "B"
3) Subject is "patchwork" and not "green fields".

Well for me the choice was between 1 & 5 as nobody will lend his name to product just becoz a campaign has been successful its generally the ppl who it affects the most who give the Name to the product or rather the name already exists.
