I am planning to take C-MAT 2013. I would like to ask a few basic questions regarding the same.

1. Is it necessary to fill individual college forms separate for call through C-MAT score (like CAT)?
2. If I have filled the forms already after CAT will I have do it again separately for C-MAT?
