The totem didn't fall, bhai. Nolan did it on purpose to leave the ending to the viewers' imagination.
There is no second reason of if the end was reality, but there is one reason of if end was a dream too. :)
Children's clothing were same in the end as they were in Cobb's dream. So, now, that means ending too was a dream.
Yes, one way to wake up in reality is to die in limbo. Cobb and Mal died in limbo (railway track wala suicide) and got back to reality.
Second way is to get the kick on any level above.
