I'll TRY to explain it, though at the moment Nolan is your man.
Mal's death was the upshot of Cobb's idea of getting back to the real world. Since he did realise that both he and Mal have been entrapped in limbo for ages ('ages' because he and Mal were shown holding hands in old age) and this was the only way to get back to the real world. So he convinced her to commit suicide (the train wala suicide) in order to get back to the real world. But the idea got so deeply mushroomed in Mal's mind that when she indeed got back to the reality, she still had the notion that she was in limbo. And that's why she committed suicide again. (Jumping from the building)
Now she always showed up in Cobb's dream because:
1) Of the guilt Cobb had in his mind; that he was the reason why Mal died.
2) Mal was part of Cobb's memory and the memory outlined the dream. So she was seen in only those dreams which Cobb had the projection of. Remember she wasn't seen in the first-two dreams, but was seen in the third dream (that snow-land wali dream).
So now to answer your question curtly, Mal was Cobb's wife who died in reality, thinking it to be a limbo. She thought it a limbo because Cobb put this idea of 'reality & limbo' way too strongly. And the reasons why she appears in dreams are aforesaid. Any other doubt, bhai?
