I'm getting only the radio buttons in EXTRA-CURRICULAR ACTIVITIES and CO-CURRICULAR ACHIEVEMENTS. How can we write in detail. What is the point in selecting only those buttons? Or there is some glitch in my form?

Admin i have changed company ones and my work experience is of around 30 month so i have to carry 30 salary slip or latest 3 will be enough?
There are 4 blank lines given beside this:
"If you are appearing for the final year Bachelor's degree Examination, indicate the time of examination
If you have already completed the Bachelor's degree Examination,indicate month/year of completion"
What do we have to fill in there and in which box??
I don't have the salary slips for all months.
Is it necessary to carry all salary slips for proof? Will only the experience certificate suffice?
Do we get to choose from multiple topics for WAT?
I have left my job , so i have the work experience certificate from my company . I have payslips for some interim months , but don't have pay slips for all the months. Since I have already left my job , wont the work ex certificate from the company and some interim payslips suffice???
