If my work ex is 3.5 months in a company, should I mention 3 months or 4 months?

We are only provided with 3 rows for the Work Ex field. If some one has worked in 4 companies, how should one enter work ex information regarding the 4th one?

I have inadvertantly made 2 mistakes in my form. I filled company sponsored category instead of self sponsored, and the other is regarding appearing in suppl. exam ever during graduation. Can someome please help me out with this. What can i do? I am ready to fill another form if reqd. Please reply asap as only 2 days are left.
