Hey adcomm! In the application form I have entered my name as written on my graduate degree whereas in CAT it was asked to enter as per your 10th certificate. The format in the two is different. In former it's 'First name/middle name/surname' while in latter its 'Last name/first name/middle name'. Please let me know what should I do now?

I want to ask what to sumbit in cgpa to percentage conversion certificate. For my 10th marksheet , the conversion factor is mention on the marksheet. so i have to upload my 10th marksheet only?. Also for my graduation, i have to upload the conversion certificate along with the 10th conversion certificate in combined form?
