Arnav, not sure how to explain - my choice is usually instinctive as I read a lot and just try to think which option I have seen used! Still will try:

(B) gives a possible confusion - do all have the same executive? or different ones?
(C) "each" is singular so shouldn't go with "their"
(D) is just structured wrong. Can't explain why, it just seems wrong to me.
(E) also felt awkward because every one (singular) should have executive of its own I felt.

I would agree with "its" over "their" here because we want to talk of each one separately. Let me give you a few examples, think which word would you use in the following:

* The RC passage had 5 questions, each with its/their own correct answer
* About a dozen of my friends came for the party, each with his/their own girlfriend.
* The numismatist arranged his coins carefully, each in its/their own special box.

I too think this ones D becuase its the inability which caused the failure
