What r d documents to b send wid the form to chetna institute for pgdm cource n also plz post d add. Where the fom to b send. (oms)

ITM navi mumbai campus is verry good nd i think 28th is d date of last gd/pi for that campus in delhi..hurry.

please some 1 guide about performa 1
the lines which is not applicable to me is to b stucked off??
and it should b written on a 100rs non judicial stam paper as i don hav indian nationality certificate..???
