I will indeed purchase it from you if it is possible for the form to be valid.
As of now, I see one option as feasible-i.e, there's a new counterfoil/slip made in my name with relavant/same date for the application number with the cooperation of the bank, and that's sent to the iim. This could be a little less likely as the logs will have been sent to the iims, though if the bank says its fine enough with this "patch" applied, it might as well be.
Another option is that the iim makes it easy for me by itself.
