I gave my GMAT this week. In my opinion -
1. Questions in powerprep are retired from GMAT sometime back. But there is a high chance that some 'similar' questions will appear in GMAT. Infact, all the questions in powerprep tests are covered in the Official Guide (OG)
2. Yes, the powerprep s/w resembles that of actual GMAT. Even Kaplan higher score CD is a close match.
3. Powerprep is not as adaptive as actual GMAT, i.e. the pool of questions in powerprep is very small, so variations in difficulty level fo questions is not that big.
